The Family Caregiver Support Program provides multi-faceted systems of support services for family caregivers, including access information, assistance, individual counseling, support groups, training, respite, and supplemental services.
Populations served by the Family Caregiver Support Program include adult family caregivers, or another individual, who is an informal provider of in-home and community care for an older adult who is 60 years of age or older, with two or more limitations to daily activity, cognitive impairment or functional dependency. Priority for services is given to older adults with the greatest need. Services include:
Help obtaining access to community resources
Counseling
Respite services as temporary support to give caregivers a period of rest
Assistance with home modifications, assistive technologies, and supplies
