Area Five offers Case Management services to individuals of all ages. Case Managers will come to your home and assess your medical needs through an assessment and eligibility tool. Programs offered are the Social Service Block Grant (SSBG), Title III, Community and Home Options to Institutional Care for the Elderly and Disabled (CHOICE), Caregiver and Medicaid Waiver Programs. Some programs are based on age and financial determination. Services offered include, but are not limited to, homemaker, home health aide, attendant care, nursing, respite, supplies, home modifications and medical equipment.
